ost people today know that the path to cavity free teeth and good oral Hygiene is to Brush and Floss daily!

Question:
Why do many people then today still get cavities?

We are living in the new millenium and technology is improving at a faster rate than ever before! Still something like the old dental cavity bugs us and we  cannot get rid of it!

Answer:
Most people do not know the correct technique or time that is necessary to maintain good oral health.

The Solution:
The solution starts with education and action or should I say the lack of it.
And finally , YES, taking RESPONSIBILITY for our own health and well-being. If we do not take care of ourselves, then nobody will! It is easy to decide not to stick to an oral hygiene routine, but actions always have reactions (ask NEWTON) or consequences.

Tongue Cleaning? What!

any of us go through the daily routine of brushing and flossing our teeth, but that’s where it ends!
The truth is most bacteria that cause BAD BREATH are embedded on the surface of the tongue.
The tongue is usually full of bacteria that is nestled in between the many folds and crevises on the surface of the tongue.

Certain conditions makes this even worst as the bacteria multiply and excrete sulphar components that is the leading cause in BAD BREATH.  Sinus problems usually cause a post nasal drip that creates a continual flow of bacteria on the dorsum of the tongue.

Morning Breath
This usually refers to the bad breath most people suffer in the morning after a nights sleep. The main reason for morning breath is dry mouth that happens during the night, causing the concentrate of bacteria and their secretions to peak.

Cleaning the tongue on a daily basis as part of your oral hygiene program will have huge benefits!
The correct instrument to use is called a tongue cleaner / scraper. If you do not have one, you may even use your toothbrush, but many people find this difficult as the bristles can cause Gagging.

No more needles?

 A needleless anaesthetic mist has been developed and could put an end to millions of petrified patient’s fears of the dentist chair. Clinical trials are expected to begin very soon on the anaesthesia that will numb the patient’s mouth without the use of the much feared painful needle injection. Reports suggest that over 145 million people of in fear of the dentist and the teeth numbing needle is to blame for the huge number of fears.
The nasal mist, called Kovacaine, numbs the upper teeth but not the lips or cheeks and therefore it is claimed that the patient won’t suffer from the usual numbing feeling after treatment. Dentist, Robert Harrison, said: “Some people are afraid of the needle, no doubt about it.
“If a dentist becomes skilled at giving injections, however, it’s pretty tolerable for most patients.” Dentists have identified the mist as an ideal replacement for needles when treating young children as it offers a pain free experience. The Pain of being a Redhead

Redheads experience more pain

Nobody likes going to the dentist, but redheads may have good reason.

A growing body of research shows that people with red hair need larger doses of anesthesia and often are resistant to local pain blockers like Novocaine. As a result, redheads tend to be particularly nervous about dental procedures and are twice as likely to avoid going to the dentist as people with other hair colors, according to new research published in The Journal of the American Dental Association.

Researchers believe redheads are more sensitive to pain because of a mutation in a gene that affects hair color. In people with brown, black and blond hair, the gene, for the melanocortin-1 receptor, produces melanin. But a mutation in the MC1R gene results in the production of a substance called pheomelanin that results in red hair and fair skin.

The MC1R gene belongs to a family of receptors that include pain receptors in the brain, and as a result, a mutation in the gene appears to influence the body’s sensitivity to pain. A 2004 study showed that redheads require, on average, about 20 percent more general anesthesia than people with dark hair or blond coloring. And in 2005, researchers found that redheads are more resistant to the effects of local anesthesia, such as the numbing drugs used by dentists.

The mutation in the MC1R gene also occurs in brunets, although it’s less common. In the latest study, the researchers tested for the MC1R gene variant, finding it in 65 of 67 redheads and in 20 of 77 people with brown or black hair. The participants were surveyed about dental-care anxiety, fear of dental pain and whether they avoid going to the dentist.

People with the MC1R gene variant had more dental care–related anxiety and fear of dental pain than those without the gene variant. And they were more than twice as likely to avoid dental care.

Dr. Daniel I. Sessler, an anesthesiologist and chairman of the department of outcomes research at the Cleveland Clinic, said he began studying hair color after hearing so many colleagues speculate about redheads requiring more anesthesia.

“The reason we studied redheads in the beginning, it was essentially an urban legend in the anesthesia community saying redheads were difficult to anesthetize,” Dr. Sessler said. “This was so intriguing we went ahead and studied it. Redheads really do require more anesthesia, and by a clinically important amount.”

After publishing research on the topic, Dr. Sessler began hearing from redheads who complained about problems with dental pain and fear about going to the dentist. He said that when someone with red hair is considering a dental or other procedure requiring an anesthetic, they should talk to their doctor about the high probability that they are resistant to anesthetics.

“Because they’re resistant, many redheads have had bad experiences,” Dr. Sessler said. “If they go to the dentist or have a cut sutured, they’ll need more local anesthetic than other people.”

Gum disease is a serious and real thread to society today and is the biggest cause for tooth loss in adults world wide. All my patients get a complete report on the health of their teeth and gums at their 6 monthly visits. Gum disease often start as the ‘typical bleeding gums’ experience many patients report at these visits. 
People that get gum disease usually do not floss their teeth and are mostly uneducated about the disease.

As you all know I got married last year and we went through the typical PLANNING and ORGANISING of a wedding. Wow, what an exciting  and stressful time it can be as so many people are involve and of course the wedding party wants to look their absolute best.
One thing for sure is that all brides and grooms and family members and who knows who want to look amazing as everyone will be photograped on the BIG DAY.  And yes, you guessed it right, everyone wants to whiten their teeth!
Before I realized what was happening I had the whole wedding party and my In-laws coming to my office for Teeth Whitening or some form of cosmetic procedure.

No Metal Please!

For many people today white fillings and whiter teeth are what they expect when going to the dentist. Many of us also still have 1 or 2 big metal fillings that was placed many years ago and for some even dates back to primary school dentistry.

Why don’t dentists like doing these fillings anymore although it seemed to last?.

 

Many of these fillings are 10 or even 20 years old and logic tells us that they therefore must be good.
These metal fillings were placed by ways of an undercut in the tooth structure. ‘Good dentists’ were given that name, because their fillings never came out easily. The bigger the undercut, the stronger the retension. The downside is that big undercuts weakens the tooth significantly.
Furthermore these metal fillings also shrink and expand over time and this leads to crack formation!

  Patients would then often blame their new resin fillings as sensitive and would even sometimes go back to metal!
Disaster strikes as the cycle would just repeat itself!

 Doctor my metal filling is sensitive.
One of the biggest complains that resulted though was that many of these fillings got sensitive or sections of the tooth just broke off, without the patient experiencing any pain. The dentist would then examine this old rusty filling and after an x-ray would just reassure the patient that no decay is present.
 No need to do anything or at least no decay is present.  Today you often still hear:  “My metal fillings are lasting very well” , but in reality the supporting tooth is badly damaged or cracked! These teeth often break while eating ‘pudding’ , ‘bread’ or who knows what.

What happens when these fillings are removed?
After careful removal of these old metal fillings, big cracks are often seen in the remaining tooth structure indicating areas of high stress.

Note the badly discoloured teeth after removal of the old metal filling. Many cracks are also present under high magnification due to the expansion and shrinkage of metal over time.

 

  
Many times these areas are just filled in again using white resin. This time a white filling is used to bond the tooth together. In the presence of cracks under the new filling, pain still exists. Drinking hot or cold drinks(temperaure difference) or biting down would result in movement of these cracks and therefore more pain.

Dentists that use ceramic fillings know that these cracks must be removed before a new filling is placed.

The new protocol:
All cracks and crack lines are removed and any blue staining from previous metal fillings are cleaned out.
The new ceramic filling needs a new solid foundation where no movement takes place.
A new life is given to the tooth.

The result:         -  No blue staining
                         -  No more cracks
                         -  White healthy new look
                         -  Zero sensitivity

So remember, next time you replace your metal fillings or have to get a big restoration, insist on ceramic.

First it was Amalgam. Yes, that is right! The old metal filling many of us still have in our mouths and that many dentists are still using. Then came gold and finally a new era of … WHITE FILLINGS!
‘White’ was always going to be the obvious colour for the ideal filling.

Ceramic, unlike other white filling materials must be made outside the mouth to obtain it’s typical charactaristics.
A good comparison is porcelain/ ceramic tiles. These tiles are not made on the spot, but rather manifactured before under very specific conditions to always produce consistant colour and other qualities.
Ceramic Fillings work in exactely the same way, but just on a smaller scale.

Here is how it works:
The dentist removes the old filling and defective tooth structure and smoothes down all sharp corners and edges.
The ceramic filling must now be made to fit exactely on and into the remaining tooth structure to complete the tooth again to its original shape. This will give us the most perfect natural looking filling available today!

IN THE PAST:
A Dental Laboratory was used to make this ceramic filling or crown.  The dentist had to take an impression of the prepared tooth;  put a temporary filling in the patient’s tooth and send the work away to the laboratory.  A week to 10 days later the filling or crown comes back from the lab and the dentist cements it in the patient’s mouth.

For many patients this is a very time consuming and inconvenient procedure.

- The 7 – 10 days  is just too long and not very cost effective
- Shots to numb the tooth on 2 different occatons in the same area

TODAY:
A milling machine was invented that can now cut the precise filling or crown out of a piece of solid ceramic!
The new CEREC 3D is here and finally dentistry has come to age!
This sort of technology is nothing new in general and is being used in the vehicle manifacturing industry for many years. Most of the parts for new motor cars are being milled out of steal from a solid piece of metal. (CAD/CAM)
 The same principle has been copied and finally Dentistry is reaping the benefits!

Since we launch the new CEREC 3D in our rooms patients are demanding white fillings more than ever before.
The new ‘white colour’ is really amazing and better ceramics and porcelains are now being manifactured with the speed of light.
For me as a dentist this is really incredible! I now have the chance to produce crowns and ceramic fillings in just one day and have the freedom of having my own LAB chairside. The quality of my dentistry has gone up by default and I am raising my own standards.
The colours I am using today are also whiter than about 1 year ago as people are becoming more focused on their SMILE as a whole and not just the tooth in question.

Wow! What an awesome weekend.  I had the privilage of meeting Dr Bill Dorfman of Extreme Make-over this weekend.
I attended a course on cosmetic dentistry which was held by Dr Dorfman.  It is great to know that dentistry in South Africa compares well with our American colleagues. Dr Dorfman is known as the ‘Best Cosmetic Dentist in LA’ and is the dentist to many Celebrities and Movie stars.

Which crown should you choose?

Dental crowns are dental restorations otherwise known as “caps” that fit over the visible tooth structure.

Dental Crowns are amazing in that they have many uses and benefits to us.

  • Dental Crowns can:
    *change the colour of a tooth
    *restore a broken down tooth
    *alter the shape of a skew tooth to align it in a straight line
    *be extremely durable and long term

  
Many of us are not aware that crowns are made from different materials and a few options exists. When your dentist wants to crown your tooth, ask him/her what type of crown he/ she would recommend.
Most people today do not know that they actually have options when choosing to crown a tooth.

These options include:

  • Acrylic dental crowns
  • Cast metal dental crowns
  • Porcelain dental crowns(cosmetic)
  • Porcelain with metal support underneath
  • Fiber reinforced resin dental crown
  • So, what do all of this actually mean to you? The bottom line is that when getting a crown done, you want something that looks exactely like a real tooth and are durable for years to come.
    Lets look at the 2 most popular crowns namely a) metal-porcelain and b) Full porcelain crown

    A) Metal-Porcelain crowns:
     * Very strong and durable
    * Typically use on back molar teeth for chewing and strentgh
    * They are very easy to cement and most dentists know how to do these crowns

    Unfortunately these crowns are responsible for the sometimes UGLY BLACK LINE you see so often when someone smiles with a big gummy smile. The metal underneath the porcelain sometimes causes staining of the gum tissues and result in a blue-ish appearance.

    B) Full Porcelain crowns:
    * These are typically cosmetic crowns and can really look beautiful when done correctly
    * The porcelain only crown let light through the tooth and therefore you get a beautiful natural look

    The 2 front teeth have been crowned showing 2 porcelain-metal crowns.  See the ‘dull’ appearance. The metal behind these crowns blocks out normal light and gives a more artificial look.

    All the teeth have been restored with full porcelain crowns. Note the natural look due to natural light shining through.

     

     Esthetics is almost always the FIRST PRICE and therefore choosing full porcelain crowns over metal-porcelain crowns almost always win.The downside to these crowns are that they can ‘chip’ more easily than metal-porcelain crowns. (This is a  small issue today, since the newer porcelains are stronger and more durable)

    The cementation technique is also very time consuming and difficult with lots of potensial problems if not done correctly. A big issue is that once they are cemented, you cannot remove them again and they have to be redone, unlike metal-porcelain crowns.

    What about price?
    Expect to pay more for full porcelain as the laborotory cost is more and like mentioned earlier, the cementation process is a lot more complicated and time consuming (dentist chair time).Whith the fast pace of new technology today, better and better porcelains are being develop and full porcelain crowns are getting more and more popular.

    No injections, please.

    Our teeth are covered with a white layer called enamel. Once through the enamel, a more yellow substance emerge, the dentine. Dentine is highly porious and consist of small tubules that are in direct contact with the nerve.

     If this yellow part is exposed, your tooth will feel sensitive in this area. This normally happens at the gum level and is coursed by hard brushing in these areas.
    When doing fillings on your teeth, it goes without saying that if the lesion is in the dentine, I have to make the tooth numb(injection) or else, you will experience pain.

    Pain Perception is highly subjective and also varies greatly from person to person. A good example is that some people might perceive an injection given to them as very painful, while others might find it absolutely ‘wonderful’. Sometimes this spectrum can vary greatly.

     Different Pain Perceptions

    Today I was once again amazed at the different pain perceptions people have. A new patient needed 2 teeth crowned and some fillings done and asked me to go ahead with the work. The surprise to me came when she told me that she does not normally take injections for dental work. Some people who do not often visit the dentist might think that this is a real normal request. Well, I can tell you that if you do a crown preparation on a tooth, it takes at least 10-15 minutes of drilling time and all this work takes place in the dentine! Yes, the dentine is in direct contact with the nerve as I explained to you before. ..OUCH!
    Now, you can probably behave bravely for 2-5 seconds, but not for 15 minutes!
    I can assure you that the teeth still had nerves in them and this lady sat through the whole procedure not blinking once.
    About 95% of people cannot do this and I don’t expect them to do this either! To be quiet honest it is torture!
    Obviousely she felt pain, but it was not the kind of pain that  warrant an injection for her.

    Her reason:  ‘She doesn’t like the numb feeling afterwards’.   Well, I told her I would write something about her today as I do not see this very often.
    As you can see dentistry is weird, amazing, fun and exciting! The list can go on…  I love my job. I get to see people from all walks of life and they let me into their personal space, even if it is only for a moment. I am truly blessed to be able to make a difference even if it is fixing a tooth. Mostly it is a SMILE. 
    To you it may not mean much, but to the person losing a front tooth, it means the world!   That I can tell you, is amazing. I can ‘literally’ put a SMILE on someone’s face! …  and I love it!

    Dental Make-over Competition

    Last week was yet another week where I saw so many patients that needed to have some Cosmetic Dentistry done , but for some reason just don’t know of the many options that are available. So many patients still think that they cannot do anything about their skew front teeth, except having braces. Braces is the last thing on their minds, so they then just accept their fate. Not that their is anything wrong with having skew teeth. It is just that they become so insecure and start living their lives with a ‘tight-lipped’ smile.  I am here to tell you that it doesn’t have to be like that! Cosmetic dentistry can help!

    Finally I have decided to do something. I am going to help someone to get the smile of  his/her dreams and everything is on me! I want to give every one a fair chance to get the SMILE they always wanted.

    How am I going to do this?
    I am going to launch a competition where anyone can enter. A Dental Make-over competition to give some one the chance to have a moviestar SMILE! The best news is, it will cost you nothing! I am not sure how I am going to do this yet, but this way I can also give something back !
